• Three, readable photocopies of the final 2008-2009 report cards.
• A check (or credit card) for Raffle Tickets -- $80/book
• A check for the Uniform/Volunteer Deposit -- $100

Flag Jerseys will be distributed at a later date, but we still need
your Uniform/Volunteer Deposit as well as your Raflle money.


WE WILL ALSO BE HAVING SNACK STAND SIGN-UPS
on Uniform Distribution dates.


THERE WILL BE A LATE REGISTRATION OPPORTUNITY
FROM 6PM to 7PM, FRIDAY JULY 3rd at the DEN.

You need to bring the following to registration:

1. A check for the registration fee ($110 per participant, $50 for Flag).
Please bring a separate check for Boys and Girls (if registering both)
ALSO, we will be accepting credit cards.

2. A recent wallet sized photo

3. For NEW participants -- An original Birth Certificate with
raised seal and one photocopy

USA FOOTBALL ONLINE COURSE:

Below are instructions on registering for the USA Football / Pop Warner Online Training Course:

1. Go to this webpage: http://www.usafootball.com/pop_warner

2. Completely fill out the registration and then enter your home zip code (Click Search)

3. Select Your League from the Drop Down list (100 mile radius of zip code)

4. Select your Association from the Drop Down list

5. Select Your Membership Type: Full Membership or CCEP (Course access only)

6. Click Continue to Payment

Important Note: One payment is complete, each coach will get an e-mail to verify their e-mail address
In that e-mail, they will click the Active Now link and be redirected to their live membership.
Verification must be complete before they can access the site.
If they do not receive the verification e-mail, they should call USA Football Member Services
at 877-5FOOTBALL.

ANYONE INTERESTED IN COACHING FOR THE 2009 SEASON...
You will need to:

1. Fill out the 2009 Volunteer Application (CLICK HERE) and PRINT & SIGN
the LAST PAGE of the Coaches Rules/Regulations (CLICK HERE)

2. Make a copy of your Drivers License (must be submitted with
Volunteer Applicaton).

3. PER NEW 2009 JERSEY SHORE REQUIREMENTS, you need to show
either your original Social Security Card, or another document
confirming your Social Security number (such as a copy of
your Tax Return or a Social Security statement).

4. All of the above must be submitted to the Football Commissioner
or Cheer Coordinator
